Holistic Eye Care — Wherever You Are

Mission

Trusted Eye Care, Delivered with Compassion.

PACE Optometry is a full-service, primary eye care provider dedicated to enhancing the quality of life for seniors across California. Our mission is to deliver comprehensive, patient-centered, concierge-style eye care directly to where patients live and thrive.

We partner with a wide range of senior care communities—including PACE centers, assisted living facilities, skilled nursing facilities, memory care units, residential care homes, continuing care communities, and retirement communities—to expand access to essential eye health services. By bringing best-in-class eye exams, personalized attention, and compassionate care to each resident, we aim to safeguard vision, support independence, and enrich the daily lives of every patient we serve.

We believe that every person, regardless of age, health, or cognitive status, deserves the ability to see to their fullest potential.

At PACE Optometry, we specialize in providing full-service eye care tailored to the unique needs of residents in long-term care facilities. With advanced portable diagnostic equipment and extensive experience in long-term care, our doctors bring the full spectrum of professional eye care directly to your facility—eliminating the stress and challenges that often come with outside transportation. Our team of independent, therapeutically licensed optometrists is dedicated to preserving vision, promoting comfort, and enhancing the quality of life for every patient we serve.

Our Resident Vision Program is designed to make quality eye care seamless and accessible. Working under the orders of each patient’s primary care physician, we verify medical and vision insurance for current eligibility and coordinate directly with facility representatives to ensure every patient who needs care is seen. Each patient receives a thorough evaluation of both vision and eye health. Afterward, we provide a clear summary sheet of the doctor’s recommendations along with two copies of the consultation report—one for the facility chart and one for the social services director. If further evaluation by an ophthalmologist is needed, we can connect the facility with trusted specialists from our referral network.

Eye Exams

Our doctors are licensed and insured to diagnose, treat, and manage a wide range of conditions. In addition to providing a glasses prescription to correct any refractive error, we manage both acute and chronic eye diseases such as:

  • Allergies
  • Cataracts
  • Diabetic Retinopathy
  • Dry Eye Syndrome
  • Eye Infections/Inflammations
  • Glaucoma Detection/Management
  • Macular Degeneration
  • High-risk Ocular Side Effects of Medications
  • Low Vision Care
  • Post-Stroke Rehabilitation Integration with PT/OT
  • MDS Assessments
  • Continuing Education for Facility Staff

Optical

For patients who need eyewear, we bring a wide selection of frames in various styles and colors to choose from. Our team takes precise measurements to ensure the best visual function and comfort.

  • Medi-Cal patients are fully covered for glasses, including replacements for loss, breakage, or prescription changes.
  • Private pay patients (since Medicare rarely covers glasses) receive a recommendation form outlining frame and lens options available through our private lab, pending authorization from their responsible party.
  • Custom engraving of the patient’s name on the temple of Medi-Cal glasses helps prevent loss or mix-ups.
  • Glasses are typically delivered within a few weeks, with expedited service available upon request.
  • Repair and adjustments of glasses, On-site.
  • Recycling and donation of old eyewear to medical mission trips with Authorization.

Equipment

At PACE Optometry, we provide comprehensive, on-site eye care by deploying a fully portable diagnostic suite that replicates the functionality of a traditional exam room. This portability allows us to serve bedbound patients, individuals with limited mobility, and those with cognitive impairment, reducing barriers to essential vision care.

Our approach integrates both time-tested diagnostic techniques and modern advancements in technology. Using retinoscopes, handheld slit lamps, binocular indirect ophthalmoscopes, and rebound tonometers, we perform objective, thorough evaluations of ocular health. These methods enable us to accurately prescribe corrective lenses and diagnose eye conditions—even in patients who may be unable to provide subjective responses.

By combining clinical precision with mobility, we enhance access to high-quality eye care and improve patient outcomes across diverse care settings.

Binocular Indirect Ophthalmoscope (BIO)

We use a binocular indirect ophthalmoscope to obtain a wide, detailed view of the retina and optic nerve. This allows us to detect sight-threatening conditions such as retinal tears, diabetic retinopathy, or glaucoma—even in challenging exam environments.

Handheld Slit Lamp

Our handheld slit lamp provides a close-up, magnified view of the front structures of the eye (cornea, lens, iris, eyelids). This helps us diagnose conditions like cataracts, dry eye, corneal disease, and ocular surface disorders with precision.

Rebound Tonometer

We use a rebound tonometer to measure intraocular pressure (IOP)—a critical factor in detecting and managing glaucoma. Because it works in both upright and supine positions, it’s ideal for patients who are bedbound or have mobility limitations.

Retinoscope

With a retinoscope and skiascopy racks, we objectively measure a patient’s refractive error to determine their glasses prescription. This method is especially valuable for patients who cannot verbally communicate or provide reliable subjective responses.

Manual Lensometer

A portable lensometer lets us measure the current prescription in a patient’s glasses on site. This helps us track prescription changes over time and ensure new lenses are updated accurately.

LEA Charts

Our LEA charts use specially designed symbols and optotypes to test vision in patients who may be nonverbal or cognitively impaired. This allows us to obtain accurate visual acuity feedback from children, seniors, and patients with dementia.

The Importance of Eye Exams for Seniors

As we age, our eyes—like the rest of our body—undergo natural changes. While some vision changes are a normal part of getting older, others may signal more serious eye health issues. That’s why regular eye exams are an essential part of maintaining overall health and quality of life for seniors.
